The Bolivia Public Health Passenger Locator Form is a travel document that must be completed by international travelers before entering Bolivia. This paper was created to help stop COVID-19 from spreading.
Before landing in Bolivia, travelers must complete the Bolivia Public Health Passenger Locator Form. At the airport, they must give this form to immigration agents.
You’ll need to provide some information about your health and your flight in order to apply for this document. To enter the nation, all passengers aged 5 and up must show proof of COVID-19 coverage on their health insurance as well as a negative COVID-19 RT-PCR test result. The test result must have been obtained within three days of the flight.

Important Instructions All travelers entering, regardless of their nationality, age or any other consideration must carry their Bolivia Passenger Locator Form (COVID-19) printed along with their RT- PCR COVID-19 Negative Test Results. The Bolivia Passenger Locator Form (COVID-19) is a Single Entry document. It DOES NOT replace a visa. If your country requires a visa, the immigration officers will ask for a Visa when you enter the country. Keep in mind some nationalities will require a paper visa. If that is your case, we suggest you contact your local embassy. At the airport, you must show printed the Bolivia Passenger Locator Form (COVID-19) that we will send you through email, in order to pass the health check established by the Health Authorities. All travelers arriving in Bolivia must quarantine for 10 days at designated facilities at their port of arrival.
