The form is one of the safeguards in place to ensure that foreign travel is secure. Passengers can be notified if someone they travel with later tests positive for COVID-19 by giving contact information and flight information.
Tracking visitors from other countries helps to reduce infection rates and makes the India a safe destination for both inhabitants and visitors.

Important Instructions Air Suvidha Self Declaration Form To be Mandatorily Filled By All International Arriving Passengers to India. All International arriving passengers are required to fill the Self Declaration form as mandated by the Ministry of Health & Family Welfare. Air Suvidha is an online system for International passengers to submit a mandatory Self Declaration form to declare their current health status. This form is mandatory and should be filled in anytime before boarding. Air Suvidha is a contactless solution by the Ministry of Civil Aviation and Delhi Airport for all international passengers coming to India. It is a Self Declaration and Exemption Form Portal for International arriving passengers. Passengers are required to mention Flight number, Seat number, Date of arrival and Country of Departure (origin) while filling up the online Self declaration form. These are mandatory fields.

The situation in each country during the Covid 19 pandemic is changing rapidly. Please check with the foreign office of the country in order to ascertain the most up to date entry requirement for vaccination and Covid tests.

You can enter India, via your Medical eVisa, on any date within the period it’s valid.

If you reside in or you’re travelling from a yellow fever-affected country, you’ll be required to present proof of vaccination against the disease.

An official document that enables you to enter and travel in India, a Medical eVisa can be obtained for entry and travel specifically for receiving medical treatment at a reputed/ recognised hospital or treatment centre in the country.

You’ll need a hospital letter (outlining the medical purposes of your visit), a digital copy of the last page of your passport, a digital passport-sized photo and a credit/ debit card or PayPal account (for payment).

Once obtained, a Medical eVisa is valid for 60 days, beginning the date of your arrival in India, and enables you to enter the country a total three times within that 60-day window. However, be aware that a completed Self Declaration Form is valid for just one single entry to the country.

Your passport must be valid for no less than six months beyond the date of your arrival (and contain two blank pages for stamping purposes).

You can apply for and use a Medical eVisa for India three times in a calendar year (January-December).

Once your application has been processed, we will email you your Medical eVisa document for India. Be sure, then, to print out a hard copy of the document (and print out a hard copy of your completed and processed Self Declaration Form) to present it to an immigration official on your arrival.

The maximum stay in the country with a single Medical eVisa is 60 days.

A single Medical eVisa enables you to enter three times within its 60-day window of validity.

A Medical eVisa can be used to enter India via 26 designated airports; Ahmedabad, Amritsar, Bagdogra, Bengaluru, Calicut, Chennai, Chandigarh, Cochin, Coimbatore, Delhi, Gaya, Goa, Guwahati, Hyderabad, Jaipur, Kolkata, Lucknow, Madurai, Mangalore, Mumbai, Nagpur, Pune, Tiruchirapalli, Trivandrum and Varanasi & Vishakhapatnam. As well as the Cochin, Goa, Mangalore, Mumbai and Chennai seaports. (Medical eVisa holders can, however, leave the country by any authorised Immigration Check Post)

No, the Tourist eVisa is suitable for leisure and tourism visits to India only – it’s also not suitable for holders of emergency/ limited passports.
